New clients, please be sure to complete all required forms prior to your first visit.

Forms

Please note that the Mental Health: Full Intake Form, HIPAA Notice of Privacy Practices, and Contract for Therapeutic Services forms need to be completed prior to your first session. There may be other documentation required on a case-by-case basis. The Referral Form can be filled out at the beginning of our therapeutic association in case you wish me to communicate with other Health Professionals.

 

Financials

Private Pay

Sessions are $150 for 45 minutes. Other payment methods may be used if expressly agreed upon by the therapist and client.

Insurance

I currently do not accept insurance, and have no plans to join insurance panels at this time.

Finally, if you must cancel your appointment, please do so prior to 24 hours, as anything less than a 24 hour notice may incur payment of fees. Additionally, if you “no show” to your appointment, you will also incur payment of fees. After three “no shows” I may remove you from the schedule.

Cancellation

If you must cancel your appointment, please do so at least 24 hours prior, as anything less than 24 hours notice will incur a fee. If you “no show” to your appointment, you will also incur a fee. Three (3) “no shows” may lead to your removal from the schedule.
